Sibutramine affects chemicals in the brain that affect weight maintenance. Sibutramine is used together with diet and exercise to treat obesity that may be related to diabetes, high cholesterol, or high blood pressure. Sibutramine may also be used for other purposes not listed in this medication guide.
Sibutramine, formerly sold under the brand name Meridia among others, is an appetite suppressant which has been discontinued in many countries.

Sibutramine, chemical name N-N, N-dimethylamine, chemical formula C ₁₇ H ₂₆ ClN, is an appetite suppressant that acts on the central nervous system. Its chemical structure is somewhat related to amphetamines, but its mechanism of action is different. It can be used as an oral weight loss medication in medicine, usually produced using sibutramine hydrochloride. Variants include chlorosibutramine and chlorosibutramine. The drug was developed by Abbott Laboratories in the United States,

Sibutramine is a serotonin and norepinephrine reuptake inhibitor (SNRI) that reduces the reuptake of norepinephrine, serotonin, and dopamine in the human body, thereby increasing the levels of these substances in synaptic cleft and helping to enhance satiety. Especially for the action of the serotonin system, which is believed to affect appetite. The previously commonly used appetite suppressants, such as amphetamines and fentanyl, operate by forcing the release of these neurotransmitters rather than inhibiting their reuptake. [2]
The mechanism of action of sibutramine is similar to that of tricyclic antidepressants, and its antidepressant effect has been confirmed in animal models of depression. [3]
According to reports, sibutramine is a prodrug of two active metabolites, demethylated sibutramine (M1; BTS-54354) and bis demethylated sibutramine (M2; BTS-54505), which have stronger monoamine reuptake inhibitor potency. [4] Further research has shown that the appetite suppressing effect of the (R) - enantiomer of each metabolite is significantly stronger than that of the (S) - enantiomer. [6]
Unlike other serotonin appetite suppressants such as phenytoin, sibutramine and its metabolites have low affinity for 5-HT2B receptors.

Sibutramine is rapidly absorbed from the gastrointestinal tract after oral administration (Tmax 1.2 hours) and undergoes extensive first metabolism in the liver (oral clearance 1750 L/h, half-life 1.1 hours), forming pharmacologically active demethylated and demethylated metabolites M1 and M2. The plasma concentrations of M1 and M2 reach their peak within 3-4 hours. According to quality balance studies, on average, at least 77% of a single oral dose of sibutramine is absorbed. The absolute bioavailability of sibutramine has not been determined yet

Usage and dosage
This medicine needs to be supplied with a doctor's prescription in most areas (if not all). Under the prescription of a doctor, combined with dietary control and exercise, this medication can be used as an adjuvant therapy to control obesity, but it should not be taken for more than a year as weight gain may occur after discontinuation. When administered orally, initially take 20 milligrams daily in the morning. If the weight loss is not satisfactory after taking it for 4 weeks, the dosage can be increased to 25 milligrams per day. If the weight loss is still not satisfactory after another 4 weeks, the medication should be stopped.
People under 18 years old or over 65 years old are not recommended to take this medication.
